Privacy Policy

Center for Transformation and Innovation, LLC, a Florida Limited Liability Company d/b/a The Physician Leadership Institute (“Company,” “we,” “us,” and “our,” as applicable) is the owner and operator of www.eli.physicianleadership.org (“Site”) and the mobile app called eCTI (“Application”), both of which are used to support a comprehensive coaching system and service (“Service,” and together with the Site and the Application, the “eCTI System”). We recognize and respect the importance of maintaining the privacy of our guests, customers, and other users and have therefore established this privacy policy (“Privacy Policy”). Described below is what information we collect, how we collect it, the purposes for which we use the information, and other helpful information. This Privacy Policy is part of our End User License Agreement, which governs your use of the Application. By visiting the Site or using the Application, you are accepting the practices described in this Privacy Policy and agree to receive future revisions to our Privacy Policy through the Site or the Application. If you do not agree to the provisions of this Privacy Policy, then you are not permitted to use any part of the eCTI System. This Privacy Policy is applicable only to visitors and users of the eCTI System, and it may not be applicable to transactions or information gathered outside the eCTI System. No party of the eCTI System is directed to or intended for individuals under the age of 18.

What information do we gather?

The information collected through the eCTI System may include, without limitation:

  • Personal Information You Give Us: We receive and store any personal information you enter through our Site, the Application, or in any other way. For example, when you register for an account or engage in our eCTI System, we will collect certain information that may include, without limitation, your name, address, telephone number, email address, or other information. We use this information to enhance and customize services and features for you, to improve the eCTI System, to communicate with you, and to offer you customized service or features.
  • Automatic Information Collection: From time to time we may use third-party management and analytic products and tools to gather certain information about your use of the Site or Application. The products and tools may include cookies, beacons, JavaScript, and other technologies. We may also collect information such as the pages of the Site or Application that you visit, your operating system version, or other such information. The information we collect enables us to run Site and Application usage tools and create statistics about our eCTI System. These statistics help us to better understand how our eCTI System is being used and what we can do to make it more useful to visitors and users. We may also collect information about your computer, mobile device, or software to alert you of any technical or compatibility deficiencies that may disrupt your use of the Site or Application. This automatically gathered information is neither collected nor stored in a personally identifiable format.
  • Minors: We follow the federal rules, restrictions, and guidelines of the Children’s Online Privacy Protection Act, under which parents can control what information is gathered online about their children. We do not knowingly collect, maintain, or use personally identifiable information about persons under the age of 13.

When do we collect information?

From time to time, the Site or Application collects personal information online, such as when: (i) you register an account with us, or engage our eCTI System; (ii) you submit content to us; (iii) you submit questions or comments to us; or (iv) you voluntarily change your profile information. This information is collected through our Site and Application, through your email correspondence with us, through your interaction with the eCTI System, or when you modify your account information through written or telephone communications. The information you enter might be collected even if you cancel or do not complete the intended use, registration, application, or submission process.

What are “cookies” and how does the Site use them?

Cookies are small pieces of information or data stored by your browser on your mobile device or computer’s hard drive, at a web site’s request. The cookies, web beacons, or other related identification devices used by our Site or Application do not gather any personal information, and these cookies are used to: (i) remember you when you login to one of our Site or use the Application; (ii) remember your settings and preferences; (iii) help us understand the size of our audience, traffic, and bandwidth patterns; and (iv) manage Site and Application information and graphics displayed on your computer or mobile device. If your browser refuses to accept cookies, you may experience browsing difficulties or the inability to log in to your account or apply for products and/or services online. Refusal of cookies may disrupt or disable other features of the Site or Application, such as the security features that automatically terminate your login session if you forget to logout.

What is Google Analytics?

From time to time, we may use analytical tools, such as Google Analytics or other tools, to collect information about use of our Site or Application. These tools collect information such as how often users visit our Site or use our Application, what pages they visit or features they use when they do so, and what other sites they used prior to coming to our Site. We use the information we get from these analytics tools only to improve our Site or the Application. Some of our analytics tools may collect the IP address assigned to you on the date you visit our Site, rather than your name or other identifying information. The analytics tools may install a permanent cookie on your web browser to identify you as a unique user the next time you visit the Site; however, the cookie cannot be used by anyone other than the providers of the tools.

How we use the information about you that we collect?
The information that we collect from you is used to provide you with the Services, content, and features
of the Site, Application, and the eCTI System. At times we may share your information with our business
partners and affiliates who provide products and services that may benefit you. In addition, we use
information we collect to:

  • communicate with you;
  • create and maintain accounts and enforce our password policy;
  • answer your e-mail or other electronic correspondence;
  • send information that you request;
  • monitor and counteract any fraudulent activity;
  • to customized products or services that may be offered to you though our Site or Application;
  • analyze the statistical use and performance of the eCTI System and accounts to provide an enhanced and more personalized interactive experience for you; and
  • in any other way we may describe when you provide the information.

We may disclose information to third parties about your account or the transfers you make:

  •     to complete transfers or payments;
  •     to provide our Services or eCTI System to you;
  •     to our affiliates or business partners that we use to support the eCTI System;
  •     to fulfill the purpose for which you provide your information;
  •     to verify the existence and condition of your account upon the request of a third party, such
  •     as a law enforcement agency;
  •     to comply with government agencies or court orders, or as permitted by law;
  •     as otherwise provided in other agreements you have with us regarding your accounts;
  •     to enforce our End User License Agreement;
  •     if we believe disclosure is necessary or appropriate to protect our rights, property, or safety, our customers, users of the eCTI System, or others (this includes exchanging information with other companies and organizations for the purposes of fraud protection and credit risk reduction);
  •     in any other way we may describe when you provide the information; and
  •     for any other purpose to which you consent.

How long do we retain the collected information?

Most types of information we collect through the eCTI System are retained according to our document and information retention policies. However, certain types of information, such as contracts or tax related documents, are retained permanently, or as required by law. We reserve the right to change our information retention policy from time to time, so please check with us frequently for updates in how we retain information. At your request, and as permitted by law, we will delete or destroy any of your information that you do not want us to retain. However, we reserve the right, at our sole discretion, to retain archival copies of such information for our records, and our archival copies will be retained in accordance with this Privacy Policy.

With whom do we share your information?

We respect the privacy of our guests, customers, users, and other visitors. We may disclose any of your information to: (i) our vendors, employees, and business affiliates for the purpose of operating the eCTI System, providing our Services, or providing enhanced service and features to you; (ii) authorities or legal counsel when we believe disclosure to be appropriate to comply with any law, regulation, or governmental or law enforcement request, or to comply with a judicial process; and (iii) any other person or entity when we deem such disclosure reasonably necessary to protect our rights or property, our members, employees, officers, or directors. We will make such disclosures only to the extent necessary to serve our purpose.


Certain features and portions of the eCTI System may require account registration and log-in processes for which you will need a username, password, or other authentication credentials. Any such credentials provided to you may be used by you solely for your individual or internal business use of the Sites. Your use of your credentials is governed by our online End User License Agreement.

Accessing and Correcting Your Information

You may send us an e-mail at our contact information below to request access to, correct, or delete any personal information that you have provided to us. We may not accommodate a request to change information if we believe the change would violate any law or legal requirement or cause the information to be incorrect.

Consent to Transfer

The eCTI System is operated in the United States. If you are located outside the United States, please be aware that any information you provide to us will be transferred to the United States. By using the eCTI System or providing us with your information, you hereby consent to this transfer. Although we will collect and use your information only as stated in this Privacy Policy, laws generally applicable to the protection of personal data in the United States may not be as stringent as those in some users’ home jurisdictions.


In offering our coaching Services through the eCTI System, we may partner with affiliated third-party vendors or contractors that provide certain information, services, or other useful tools, and we may provide you with links to, or otherwise direct you to, the content or websites of such third-party affiliates. You should be aware that when you navigate to third-party websites, you will be subject to the privacy policies and terms of use of those third-party websites, and not ours. While we support the protection of privacy on the Internet, we cannot be responsible for the actions of third parties. We encourage you to read the posted privacy statement and user terms and conditions whenever interacting with, and prior to providing any personal information to, any other website.

Data Security

We have implemented measures designed to secure your personal information from accidental loss and from unauthorized access, use, alteration, or disclosure. All information you provide to us is stored on our secure servers behind firewalls. Any payment transactions will be encrypted using SSL technology. Although we make reasonable efforts to safeguard your personal information once we receive it, we cannot guaranty the security of information you transmit to us or that we store for you. We cannot be, and are not, responsible for the actions of our third-party vendors, or for unauthorized access to information by hackers, or by other unauthorized third-parties.

The safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to certain parts of our eCTI System, you are responsible for keeping this password confidential. We ask you not to share your password with anyone.

Unfortunately, the transmission of information via the internet is not completely secure. Although we do our best to protect your personal information, we cannot guarantee the security of your personal information transmitted to our eCTI System. Any transmission of personal information is at your own risk. We are not responsible for circumvention of any privacy settings or security measures contained in the eCTI System.

Your Consent and Changes to this Policy

Subject to the above provisions, by using any of our eCTI System, you consent to the collection and use of information by us and the terms of this Privacy Policy. We may change our Privacy Policy from time to time, and we will post those changes so that you are always aware of what information we collect, how we use it, and under what circumstances we may disclose it.

If we make material changes to how we treat our users’ personal information, we will notify you by e-mail to the e-mail address specified in your account and/or through a notice provided through the eCTI System.

You are responsible for ensuring we have an up-to-date active and deliverable e-mail address for you, and for periodically visiting our eCTI System and this Privacy Policy to check for any changes. By using any of the eCTI System after such changes and posting, you agree to be legally bound and to abide by the changes and other provisions contained in the Privacy Policy and End User License Agreement.
